If i had to pick only one I'd help my friends and family out, knowing I am taken care of I'd just live comfortably nothing to luxurious, be a philanthropist with a specific charity

First I'd make donations to get marijuana law reformed
Then most of my family gets lifetime financial security.
I'd send a check to anyone that ever made me smile. I feel it's only fair.. They made me happy so I should return the favor
Every local animal shelter around me would be setup for life.

Then I'd buy 1000 acres, and never see any of you again.

Nope, you're still not quite getting it. It's not if you had to pick only one item what would it be. It's which one thing would you do or get first? Knowing that you have enough money to do most anything your heart desires and you'll be able to do all the other things you want, the next day, week, month, year.
See most everyone says, I'd get a nice house, car, bike, bling, stuff, etc. and with over 100 Million anyone would be able to do all that no problem. So a list of the things you would want doesn't really say much about a person. However, what one thing a person would place above all else does say alot about a person.
I would get a new set of lungs,because I have destroyed them by smoking Fn cigs.

You didn't really answer the question I asked. You answered the question of what would you do if you won a mega million lotto. I asked what would you do first. There's a subtle difference. Now to your credit you did say you would do things in that order but it just didn't seem realistic that you would hold off on doing other things you could do just to pay off your and your family's debts. It would probably take a few days at least to gather up all the debts and set up a way to pay them off and during that time there are other parts of your master plan that could be put into motion. Also being that it is a whole a plan you could say given the amount of wealth we're fantasizing about that it is something that wouldn't necessarily have to be done first.
I chose "take care of medical needs" first because there's so much more I would want to do I'd want to have my health all squared away so I could focus on enjoying everything else that would come afterwards, houses, cars, bling, women, parties, drugs, etc.
What would you do?

If I won the lotto for say $100 million, Id honestly give about half of it away to charities. Well I think id actually create my own, because you really dont know where your money is going when you donate to charity. If i had that much money it would be my demise.
