Can my family dr do my renewl?

There can't be any changes if you want your renewal to be approved and it must be sent in before the expiration of your current license.

I would suggest you give the MMAR a call(1-866-337-7705) and ask them.
I was in the same sorta position...My Doc had moved to another address. I was told the changes they where concern with were your information(change of address, amounts, etc., etc.). The only way to know for sure is call HC directly(once again...1-866-337-7705). It's always best to get your information as close to the source as possible.

PS. I was pleased at the time my renewal came back this time, sent it in 12 weeks early(Beginning of Nov./13) as always, received it Dec. 6/13, it was always late, a month to 6 weeks after expiration date. To bad, it might be all ending next spring
