How to sign up a patient?

Greetings fellow mile high'ers. I am in the process of becoming a caregiver, and I was wondering what the patients' paperwork needs to say, as far as designating me as the caregiver. I imagine it needs to go along the lines of "I, So and So designate So and So to be my caregiver" notarized, dated...yadda yadda... So what else do the papers need to say? Thanks ya'll.

your patient needs to fill this out in blue ink, get it notarized, and include a photo copy of your drivers license

i always go down to the department of health and make sure everything is right with one of the people that work behind the window. it takes way longer than just mailing it in but you at least have another set of eyes looking at your stuff before you mail it.

They are no longer allowing us to bring it to the window and have it processed faster. That is what actually caused the backlog. I spoke with Debra a few months ago about it. Mailing and walking it in is now the same length of time.I would also put together a contract for patients to abide by as well as you.

contracts do not work, won't hold up in court, Patient hold the rights to say do what they wish, Caregiver is at the mercy of the Patients Wishes.

You may want to talk to a lawyer about that. You are way wrong, many have been held up in court. As a matter of fact Rob Corry has ones available for cg's. Also you may want to re-check stuff,patients have no rights especially when it comes to growing, if they did we would have a patients bill of rights and we cg's would have rules we would have to abide by. Go to a MMBA or Senate Meeting before saying such absurd things. I am a patient and cg so I have to stay up on both rules.

So if johnny law shows up at my grow, I should probably keep copies of all patients paperwork, since it will take 8 months for the state to even acknowledge that I have legitimate patients. Gotcha. Whats the deal with the edibles license? What special conditions are needed to have that privilege and what are the perks of having one?

All i keep on hand are notorized copies of the change of caregiver form.

Are you talking about the infused product license or just the higher plant count for edibles some patients have?

I mean the higher plant count, I'd like to have that ability as well as my patients. I want to do more smaller plants, so I'd like to be flexible on my plant count.

Well until the actual case law gets sorted out what we are working with is this(to my understanding)...

say your patient has a 24 plant count...under the old law that didnt mean crap. Under the new law that gives you an arbitrary defense. It all really depends on the county you are lucky enough to be apart of. All you have to do is call the DA of said county and ask them if they are going to be recognizing higher plant counts for patients under the new law.....

In order for a contract to be valid, the contract can not be in violation of any laws, including federal laws. At least that is my understanding

Amend 20 is a contract between the state and the people and is a direct violation of fed law. So it has no bering, I have spoken with a few big lawyers on this and if it is agreed upon and signed by both parties it holds up in court.

on the state level yes, but beyond that..I don't think any contract that has to do with mmj will hold up.
