This is a picture of a character from the film Lord Of The Rings: Return Of The King called, "The Mouth of Sauron" that never made it off the cutting room floor. Supposedly, this person had spoken so much evil in the course of his life that his mouth was permanently disfigured by it - this is creative license. The original documents by the author (J.R.R. Tolkein) make no such distinction.
